How to Complete an Amended Tax Return

    • 1). Write your name, address, Social Security number, and spouse's name and Social Security number (if applicable) in the first part of the 1040X form.

    • 2). Complete sections A. B and C. Enter your filing status whether or not you are changing the filing status. Select the year the original tax return was for and explain the changes that you are making to the federal tax return and why you are making the changes.

    • 3). Enter any changes to income and deductions in the section under "Income and Deductions." For instance, if your original return has an adjusted gross income of $25,000 and you are adding a W-2 with income of $2,500, the amount that you enter on line 1 would be $27,500. Complete all lines that apply. If you are changing exemptions, you need to complete page 2 of the 1040X form.

    • 4). Make any corrections to your tax liability under "Tax Liability." If you have changed any amounts under the first section, "Income and Deductions," you may change your tax liability and credits. If you are changing any credits, use the correct worksheet listed on the original return to recalculate using the new information.

    • 5). Enter the correct amounts for any payment or refundable credits under "Payments." Use any Schedules listed on the original federal return if the credits and payment have changed.

    • 6). Complete the last section to see if you will have a refund coming or if you will owe money. Sign and date the amended return. Attach any supporting documents for the changes that you have made including new Schedules for deductions or credits. Mail the return to the corresponding address for the state that you live in found on page 5 of the IRS: Instructions for Form 1040X (see Resources).

    • 7). Complete a state amended return if you had changes to the adjusted gross income, credits or deductions.
